ApicHope’s AR882 Beats Febuxostat in China Phase 3 Gout Trial, Meeting Primary Endpoint

China’s ApicHope Pharmaceutical Group has reported positive topline results from a Phase 3 clinical trial of AR882 (pozdeutinurad), its next-generation URAT1 inhibitor for gout, showing statistically significant superiority over febuxostat in lowering serum uric acid (sUA).

According to ApicHope Pharmaceutical Group’s corporate announcement, the randomized Phase 3 study enrolled 683 patients with gout and hyperuricemia in China and achieved its prespecified primary efficacy endpoint.

At Week 24, 86.0% of patients receiving AR882 75 mg achieved serum uric acid below 360 μmol/L, significantly outperforming the febuxostat 40 mg control group (P<0.001). (Sina Finance)

The result provides late-stage clinical validation for AR882 in China and could position the investigational therapy as an important new urate-lowering option if ultimately approved.

Strong Uric Acid Reduction at More Stringent Targets

The Phase 3 trial was designed as a multicenter, randomized, double-blind, parallel-controlled study comparing AR882 capsules with febuxostat in Chinese patients with gout accompanied by hyperuricemia. A total of 683 participants were enrolled. (Sina Finance)

The efficacy results at Week 24 were:

Importantly, AR882 demonstrated statistically significant superiority not only at the conventional target of <360 μmol/L, or approximately 6 mg/dL, but also at the more stringent thresholds of <300 μmol/L and <240 μmol/L. (Sina Finance)

ApicHope has not yet disclosed the corresponding response percentages for the febuxostat arm in its topline announcement. Therefore, the magnitude of the treatment difference between AR882 and febuxostat at each threshold cannot yet be fully assessed.

Safety Profile Appears Favorable

AR882 also showed an encouraging safety and tolerability profile in the study.

According to the company, the majority of treatment-emergent adverse events (TEAEs) were mild to moderate, and there were no cases of permanent treatment discontinuation, study withdrawal, or death attributed to TEAEs. (Sina Finance)

More detailed safety data—including individual adverse events, laboratory abnormalities, renal safety measures and rates across treatment groups—have not yet been disclosed in the topline announcement.

These details will be important when assessing AR882’s overall benefit-risk profile, particularly because urate-lowering therapy is generally used chronically.

What Is AR882?

AR882, also known as pozdeutinurad, is an investigational, highly selective inhibitor of urate transporter 1 (URAT1).

URAT1 is an important transporter involved in the reabsorption of uric acid in the kidney. By inhibiting URAT1, AR882 is designed to reduce renal reabsorption of uric acid and increase its urinary excretion, thereby lowering circulating serum urate levels. (Sina Finance)

The drug has been developed through a collaboration involving ApicHope and U.S.-based Arthrosi Therapeutics. The companies established Guangzhou Ruianbo Pharmaceutical Technology to develop AR882 in Greater China, while Arthrosi led its broader global development program. ApicHope currently holds 100% of the market rights to AR882 in Greater China, including mainland China, Hong Kong, Macau and Taiwan. (ApiChope)

Building on Earlier Clinical Evidence

The China Phase 3 success builds on earlier clinical studies suggesting that AR882 can produce sustained reductions in serum urate and potentially reduce the burden of urate crystals and tophi.

ApicHope previously reported that in a global Phase 2 program involving patients with difficult-to-treat tophaceous gout, treatment with AR882 alone or in combination with allopurinol produced reductions in serum urate and urate crystal burden. At 12 months, 43% of patients receiving AR882 75 mg and 57% receiving AR882 75 mg plus allopurinol had complete resolution of at least one target tophus, according to the company. (ApiChope)

AR882’s global development program has also progressed into pivotal Phase 3 testing. The REDUCE 1 and REDUCE 2 trials are evaluating the drug in patients with progressive and tophaceous gout. ClinicalTrials.gov’s AR882 Phase 3 study information

Both global pivotal trials had completed enrollment by 2025, underscoring the advanced stage of the international development program. (ApiChope)

A Potential New Competitor in the Gout Market

The Phase 3 result is particularly noteworthy because AR882 was tested against febuxostat, an established xanthine oxidase inhibitor used to lower uric acid in patients with gout, rather than solely against placebo.

The two therapies attack the problem from different directions. Febuxostat reduces the production of uric acid by inhibiting xanthine oxidase, while AR882 is designed to increase the renal excretion of uric acid through URAT1 inhibition.

Demonstrating superiority over an active urate-lowering therapy therefore strengthens the clinical case for AR882, although full interpretation will require the complete Phase 3 dataset.

The proportion of AR882-treated patients reaching increasingly stringent urate thresholds may also be clinically relevant for patients with substantial urate crystal deposition or tophaceous disease, where deeper and sustained urate lowering can be particularly important.

Regulatory Path Comes Into Focus

The positive China Phase 3 results potentially clear an important clinical hurdle toward a future regulatory submission in China.

ApicHope said it plans to communicate actively with Chinese drug regulators to advance AR882 toward a marketing application. However, the company cautioned that AR882 remains investigational and will require regulatory review and approval before it can be commercially marketed. (Sina Finance)

The drug’s global development story is also significant. AR882 has received Fast Track designation from the U.S. Food and Drug Administration for the treatment of visible tophi in patients with clinical gout, while pivotal global studies continue to evaluate its potential in progressive and tophaceous gout. Taken together, the China Phase 3 results provide another major validation point for AR882. If the detailed dataset confirms the topline efficacy and safety findings—and ongoing global Phase 3 studies are similarly successful—the highly selective URAT1 inhibitor could emerge as an important new oral treatment option for patients who require stronger urate lowering than existing therapies provide.
